*NOTE – 1. Use of the Worship Center Auditorium requires booking members of Liberty’s Worship Arts Production Team to run A/V for the building. This will need to be coordinated with Pastor Trey (Worship and Production Pastor) to ensure availability of available Production Team members.
2. For Liberty Ministry Events there is a 130 event attendees minimum for use of the Worship Center Auditorium.

By submitting this form, I and/or the Event Coordinator takes responsibility for the facility/resources/vehicle being used, therefore I understand I need to take care of, return clean, refuel, and otherwise leave the facility/resources in the manner in which I found them or better. I and/or my ministry assume the costs associated repairing or replacing any damages to Facilities or Resources of Liberty and I agree to pay the required Usage Fees for this Request of Use.

I understand that submitting this form is only a request and that all requests must go through a Pastoral approval process. Preference is given to Liberty Ministry Events on a first come, first served basis followed thereafter by non-Ministry requests in terms of priority. Liberty Pastoral Staff reserves the right to reschedule/cancel any approved Resource Request as needed.
